amarok-2.2.1-46.2
opensuse 11.1
KDE 4.3.4 "release 2"
mysql-5.0.67-12.16.1

I did a fresh amarok install on a few machines and every time I have to do a full rescan after a full scan and after I see all files in "Local Music". Is this a known bug?

Yes, this is a known bug, already fixed in 2.2-git (e.g. the development branch).

Normally, one full rescan and a restart should be enough, there was a database update between 2.2 and 2.2.1

Running Kubuntu 22.10 with Plasma 5.26.3, Frameworks 5.100.0, Qt 5.15.6, kernel 5.19.0-23 on Ryzen 5 4600H, AMD Renoir, X11
FWIW: it's always useful to state the exact Plasma version (+ distribution) when asking questions, makes it easier to help ...
