I know you said to open a bug report for the windows that didn\'t have the icon but seriously I can\'t be bothered currently. It is a slow tedious process with lots of annoying questions *grin*.
I think all the windows have one anyway(Ok, so I wrote this before checking -- sue me).
My list of windows and if they have a window title icon. Let me know if I missed any.
Some of these may be kde specific and not amaroK.
