Sometimes one shortcut, sometimes two

I've got a weird bug I can't figure out.

In the start menu/app launcher on the bottom left of my screen, I open it and about half the time I have two identical firefox shortcuts and other times I open the launcher I only have one. Anyone know what the heck is going on? I only have one firefox on my system, not two. These two icons appear next to each other in the list and they appear both in the "Internet" category as well as the "All Applications" category. When the one category has a duplicate link, the other does too. When the one category only has one firefox link, the other will too. I haven't been able to figure out why it has one sometimes and randomly has two other times. I also haven't figured out how to remove shortcuts from this application launcher menu despite spending literally all day long on the internet looking. Right clicking on the link will let me add it to favorites and the task manager but it doesn't have a delete option. It should.

How do I prevent this second shortcut icon from appearing? I only have one firefox install. And how do I delete shortcuts from here?
