Well, I do not have any /apps subfolder in /usr/share/kde4:

if you have the findutils-locate package installed you can run the command locate Blackbox.png
in openSUSE (some of) the icons are in the kdepasswd package, if you do rpm -ql kdepasswd|grep blackbox it should show you the path in your distro |

I just did not know the names of the icons, otherwise they would be easy to find.
So they are actually in /usr/share/apps/kdm/pics/users, as montfras mentioned. |

different distros package and locate things differently sometimes, making some answers correct for some but not all installs

KDE 4 itself installs things into $PREFIX/share/apps/<kde app name>/
OpenSUSE for unknown reasons shifts this to $PREFIX/share/kde4/apps/<kde app name>/
