I found! You icon theme is "Tango".
This theme is very poor in therm of number of icons.

That's why, many icons are missing.
I'm investigating how I can improve this.

No, this is not important. The only reason of this issue is that you are using a 'poor' icon theme. I'm adding an alternative icon for each icon in case of the preferred icon is not found. This is a big job but the result will be better even if not as good than with breeze icon theme.
