Although I've selected 'Activity By Date, Flat' by emails are grouped by day. I prefer a straight list so how do I remove the group headers.

I think the grouping is done by the "Activity" part. That said, you can try configuring the headers by selecting View > Message List > Theme > Configure....

(You may need to make a copy of the current header theme from the same interface before being able to modify it)
