kmail accounts derived from maildirs

I'm running kmail 4.7.2 under Kubuntu. When I add an account using the Accounts page of Configure, two of the alternative types are Maildir and KMail Maildir. These don't resemble Pop3 accounts at all and I can't find any documentation on them. Kmail Help seems not to know they exist, and I've been unable to figure out their behavior by experimentation. Here are some of the questions I have about them:

1. Local Folders seems to be an account of this type. It contains, among other things, the outbox folder, and kmail appears to attach special significance to this folder, since messages placed there are sent out. So how does an account qualify to be called Local Folders? For any other maildir-derived account, the name of the account is the name of the folder from which it was constructed; for the Local Folders account, that is not the case.

More generally, how does the Mail Dispatching Agent, which I guess is part of akonadi, determine where to look for outgoing mail? Clearly, not just any folder named outbox qualifies.

2. Aside from Local Folders, I have two other maildir-derived accounts. One of them shows up as a top-level folder in the Folder List on the left side of the kmail window, but the other does not. What might account for the difference?

3. What is the difference between an account constructed from a Kmail Maildir folder and one constructed from an ordinary Maildir folder? In fact, how does a maildir folder qualify to be considered a KMail Maildir folder?
