Recently this has begun to appear when I look at the Firefox download list and select an item and ask to Open Containing Folder.
I am running Kubuntu 14,04 and keep it updated. It is possible that I have not looked at a file like this in the past.

System Activity foes not show a likely candidate although akonadi_agent_*changes by one or sometimes two lines. There are a number of these instances and it is not easy to see which one changes.

And I just discovered that when I am looking at an image this same app pops up just with a different title. Now this is relatively new. I have looked at images before and gwenview wa always the default app.

That looks like the Plasma Active File Manager. Please open System Settings > File Associations, and search for inode/directory and check the associated applications. Dolphin or Konqueror should be at the top.

You should also check in System Settings > Default Applications > File Manager, to ensure Dolphin or Konqueror is set as the default.
