Setting up multi-drive system

Hi, I'm attempting to set up KDE Neon using UEFI and I'm having trouble judging on what to do for manual partitioning. I am planning on using my SSD for the Root file and any boot loaders, and a hard disk for the Home file and a swapfile. I have done this before with Manjaro, but the menus for manual partitioning are different and I'm not entirely sure how I'm supposed to go about this.

Edit: Forgot to mention, I mostly just need help setting up the boot file, as Manjaro had simplified the process of booting using UEFI.

I've done this, it's exactly how my system's configured so it can be done. I have my m.2 SSD set up as the root drive with boot loader as well as swap and I have a larger capacity mechanical hard drive set up as /home.

Sadly I didn't take a single photo of what I did and can't remember the process off the top of my head. I'll do it again on a spare PC and note down the process for you is you still haven't worked it out.
