I think there were multiple entries defined for the same instance/activity/screen/whatever. This can occur if you (un-)plug a monitor or loose connection, copying old configurations after a system upgrade/fresh install, and a lot of more things. It is not easy to tweak and I also do not know what exactly happens under the hood.
