Dual monitor setup help

Hello,

I have a systemd based Sabayon 64 bi,t up to date (16.11), installed on a Asus N76V laptop.
It has Intel HD 4000 and nVidia 650M. I have banned nouveau and installed nvidia driver.
I have a DELL U2713HM external monitor connected by HDMI to DVI adapter.
I have two issues:
1. I don't know how to make the DELL monitor to run at 2560x1440@60 - it reports on HDMI only full HD, but I know the hardware supports it fully in WIndows with custom resolution defined in nVidia control panel.
I've tried to setup it like in WIndows:

It complains at this point about not having the right frequencies (I've set in xorg.config the frequencies range from the monitor's manual).

2. I can't have KDE desktops on both displays - x.org seems to ignore my configs. I get the external monitor full HD, with KDE on it (!) and laptops builtin monitor always shows a console - the KDE's session output. If I press CTRL+F2, it stops the output to the external monitor and enables the notebook's one to login into new session.

How could I make it run on two desktops, ideally at native resolutions?
What are the prerequisites and procedures?
Can anybody share some setups?
