I'm using openSUSE 11.1 (32-bit) with KDE 4.2. I had the same system running as 64-bit two days ago, but had so many compatibility problems, that I decided to go back to 32-bit. Anyway, I did not have this problem in 64-bit, but it's showing up now in 32-bit.
When I install an application, I go into the menu editor and create a menu entry for that app. No problem. However, when I select the icon, if the icon name is icon.xpm, the icon does not show up in KDE's menu. Intead, I get the question mark. If I change the name of the icon, and point the menu item to this newly named (but still the same) file, it works fine. I also tried saving the same icon as a .png file (icon.png), and it worked fine that way. Like I said, this wasn't a problem when I was running this 64-bit version, but it is in the 32-bit version. It seems like a configuration/setting file for KDE might be messed up, but that's an uneducated guess. Any suggestions (other than rename all of my icons)? |

When you set the icon to the icon.xpm file, is the path in the corresponding *.desktop file correct?

Yes it is. It took a while, but I figured out where to find those files and I tried manually editing them. The path was already correct in the file. |

This appears to be a bug in KDE. Please update to KDE 4.3, and if that does not resolve the issue, file a bug report at bugs.kde.org.
Make sure to run "kbuildsycoca4 --noincremental" and log out and in again after changing the icon in KDE 4.3 to clear out any caches, etc.
