The graphics seem to get messed up a bit, and clicking the top most program name links it to the bottom empty slot. Seems like the bottom application overlays and rolls over to the top.

When I disabled kwin effects and re-enabled, it was fixed. ???

Which version of KDE are you using?

This issues seems to be this one here: https://bugs.kde.org/show_bug.cgi?id=261479
And should be fixed in 4.6, at least I don‘t encounter it anymore :P Or, well, maybe in 4.6.1 since you‘re using 4.6

Please file a bug report at bugs.kde.org regarding that, and include a list of applications and conditions needed to reproduce that (such as the icon being hidden, etc)

This only happens under some circumstances, I am not sure why it happens. It seems applets rather than actual applications are far more likely to trigger this issue. You may wish to alter the System Tray settings to force the display of applets residing in the System Tray.
