Unify Sound Preferences and Phonon device configuration

I went through some trouble with this, so I'll contribute a little bit.

When I was watching a movie with VLC last night, I had some trouble configuring the default sound device when I inserted the HDMI cable (I don't do this often, that's why it never happened with me before). In GNOME and other GTK-based DE's, when you select the default sound device, all sound output goes to that device. In KDE, that's not true. Sound continues playing on whatever device it was. While this is OK, because it's desirable in some scenarios, it would be nice to add a dialog prompting the user if he/she desires to move all sound output to the newly selected device.

Then, I had problems because, for whatever odd reason, the sound device selection wasn't applied to Phonon, which has its own configuration. I understand that this would be desirable in the old days, but now we have per-app sound output device selection capabilities in Sound Preferences/our standard mixer. I fail to see a reason for Phonon to have its own device selection configuration, since we can just redirect whatever app is using Phonon to the desired device. The way it works now adds unnecessary configuration and complication, resulting in a scenario most regular users wouldn't be able to figure out how to fix.

Also, for some reason KDE fails to detect the correct HDMI port a device is connected to, something that does not happen in GNOME or Unity. This falls in the Support category, but it's something that might be fixed while doing this work.

I myself would be capable of fixing these things, but I would need some guidance.
