Just to clarify: I don't think this was ever planned - it's simply a mockup.

It would be definitely possible.
There are also window-explosions or crash-sounds.

I wrote DrKonqi2 for KDE4.3 and we try to not show crashes as complete disasters (with full explosions sounds :P). We show them as errors and guide the user to the reporting process with lot of helps and options to restart the application.
We are currently using a (self-made) "broken monitor" as icon, but we need a better one.
AFAIK, when one application crashes or hangs, it stops redrawing itself (and it is closed inmediatly) so I don't know if that "broken window" effect is really possible.

KWin could do it. But it's not really useful...

Neither is putting snow on the desktop, it just looks cool.

I think that would be kind of nice, like Windows Vista fades out the window of a program that's not responding, there should be some visual cue that the program's not working. A cracked window is a bit much though! :)
